All forms are required BEFORE your child’s first day of care.

Required health forms:

(1) Current Vaccine Record meeting the Texas State Minimum Standards for Child Care Centers > Please note: The Texas Dept. of Health now requires that all vaccine records are validated with the doctor's signature or stamp. On-line patient portal records CANNOT be accepted without proper authentication.

(2) Signed Health Statement from Child’s Doctor > The health statement does not need to be on our form. You may use a standard health statement provided on the doctor’s form.

If your child is 4 or 5 years old by September 1st, your child is required to be tested for both Vision & Hearing during the 2024 calendar year.

Vision & Hearing Screening > ALL 4 & 5 year olds. Please DO NOT submit records from 2023. Screenings must be conducted in 2024. If your child has already been tested, you can either email the results or use the Health Paperwork Upload.

We will have a Vision & Hearing Screening day on Thursday, September 26 for your convenience. We encourage you to pay to have your child tested at RBCS so that we have the results needed to fulfill the requirement. If your child will be tested at their well check this fall, please complete the Vision & Hearing Screening form and let us know the date of their appointment. You will be required to submit the results after their testing date.

Results of Vision & Hearing must meet the state requirements. Visit the Vision & Hearing Screening form for details on what is acceptable. “Pass/Fail” is not sufficient to meet the requirements.

ALLERGIES & EMERgency medications

If your child has a food allergy, you will need to submit an Allergy Action Plan as well as all child’s emergency medication (unexpired with proof of child’s prescription such as prescription label or box). A new plan is required for each new school year.

If your child has another medical need that requires emergency medications to be on file, parents should provide written instructions from the child’s doctor including child’s name, date of birth, name of medication, dosage, doctor’s signature and parent’s signature. Medications need to be unexpired and in original container with prescription label.

Providing Permission for a Topical Product

Any kind of topical product such as diaper cream, lotion, lip chap, etc. requires a signed topical product permission form. You can complete the form below. Parent is responsible for providing the product (unexpired) labeled with child’s first and last name.
